Position Summary

The Connection Points Lead supports the Director of Connection Points (DCP) in implementing and sustaining all Memory Care–specific initiatives under the Connection Points Memory Care Approach (CPMCA). This role focuses on the administrative, organizational, and logistical execution of initiatives to ensure consistency, follow-through, and operational excellence within the Memory Care neighborhood.

While the Director of Connection Points serves as the outward-facing leader—educating families, coaching teams, and building relationships—the Connection Points Lead works primarily behind the scenes, managing documentation, communication systems, and initiative tracking to support the overall success and integrity of the Connection Points approach.


Connection Points Memory Care Approach – Support Role Alignment

The Connection Points Lead plays a key role in sustaining the Connection Points Memory Care Approach by ensuring systems, tools, environments, and communications are consistently maintained so residents, families, and team members experience a connected, organized, and intentional Memory Care environment.


Desired Experience for Connection Points Lead

  • Experience in senior living, Memory Care, healthcare, hospitality, or a related administrative field preferred
  • Strong organizational skills with the ability to manage multiple initiatives simultaneously
  • Proven ability to complete assigned tasks accurately and on time
  • Strong written and verbal communication skills
  • Comfort using Microsoft Office applications and digital communication platforms
  • Ability to work collaboratively and follow direction from the Director of Connection Points

Accountabilities for Connection Points Lead

The Connection Points Lead supports the work of the Director of Connection Points by assisting with the following responsibilities:

  • Demonstrates and supports company culture and values, including safety, common beliefs, and a culture of feedback
  • Provides administrative and logistical support for all Memory Care initiatives under the Connection Points Memory Care Approach
  • Supports the Director of Connection Points by organizing, tracking, and maintaining initiative documentation, timelines, and follow-through
  • Completes and maintains Resident Bios and supports ongoing updates as needed
  • Assists with updating and maintaining resident name plates
  • Drafts, formats, and distributes the Connection Points Family Monthly Newsletter as directed by the DCP
  • Supports family engagement efforts by coordinating logistics for monthly family engagement events
  • Creates and posts content on the BAND App per request and direction of the Director of Connection Points
  • Conducts follow-up calls with families as directed by the DCP
  • Supports organization, maintenance, and integrity of Interactive Environments (IE)
  • Assists with preparation and support of the self-care initiative
  • Orders and maintains supplies for Revivify and other Connection Points initiatives as directed
  • Educates on and completes SPARK requests related to Memory Care initiatives
  • Participates in meals as assigned to support and observe Family Style Dining (FSD) and the integrity of related initiatives
  • Maintains the comfort cart and all Beacon of Life (BOL) supplies
  • Assists with tracking attendance and documentation for Memory Care trainings and education sessions
  • Assists with preparation for audits, reviews, and quality checkpoints by organizing materials and data
  • Models and exemplifies the standards and expectations of all CPMCA initiatives
  • Maintains consistent follow-through on assigned administrative tasks and deadlines
  • Attends designated meetings as requested by the Director of Connection Points to support documentation, follow-up, and administrative coordination (e.g., Memory Care planning meetings, initiative check-ins, or interdisciplinary discussions)
  • Observe workflows, initiative integrity, and environmental consistency and report findings, concerns, or improvement opportunities to the Director of Connection Points, without independently initiating changes.
  • Performs other duties as assigned in support of the Director of Connection Points

Role Boundaries

The Connection Points Lead:

  • Does not lead difficult family conversations or care conferences
  • Does not make independent programming or initiative decisions
  • Does not replace the Director of Connection Points in leadership responsibilities
  • Does not perform direct resident care tasks

This role is designed to support and extend the Director of Connection Points, not duplicate the role.

General Working Conditions

This position requires frequent sitting, standing, walking, and use of standard office equipment including computers, printers, and telephones. The employee may occasionally lift or move up to 25 pounds. The work environment involves regular interaction with residents, families, and team members within a Memory Care setting. Noise level is moderate. Occasional schedule flexibility may be required to support events.
