Medication Care Manager Certification Required
Position Summary:
As a Medication Care Manager, you are responsible for providing safe, organized, and compassionate medication administration and personal care support for residents living within our community. This position plays a critical role in ensuring residents receive timely medications and treatments while promoting dignity, independence, and quality of life in alignment with StoryPoint Groups mission and core values.
Required Experience for Medication Care Manager:
High School Diploma or GED preferred. Medication Care Manager Certification Previous healthcare, geriatrics, or experience working with the elderly population preferred. Experience with Electronic Health Records (EHR) systems and medication administration preferred. Understanding of medication safety protocols and resident care standards. Desire to work with the geriatric population. CPR and First Aid certification.
Responsibilities for Medication Care Manager:
Administer, assist with, and observe medications and treatments for residents according to physician orders and community policies using the Medication Administration Record (MAR). Follow the Six Rights of Medication Pass including right resident, right medication, right dosage, right time, right route, and right to refuse. Review, read, notate, and initial electronic health records to document medication administration, resident information, refusals, and missed doses. Receive medication updates from the Wellness Director or Nurse and ensure medications are passed according to scheduled times. Observe residents for changes in physical, emotional, and mental condition and report concerns promptly to nursing leadership. Maintain strict confidentiality of resident and community information. Follow infection prevention and control procedures, workplace safety standards, and proper medication disposal practices. Assist residents with activities of daily living (ADLs) including ambulation, toileting,
dressing, grooming, and personal hygiene as needed. Respond promptly to emergency call systems and resident needs. Assist with resident transfers, positioning, and mobility while utilizing proper safety procedures and equipment. Help maintain residents dignity, privacy, and independence while fostering meaningful relationships with residents and families. Ensure medication carts, supplies, and resident care equipment remain clean, organized, and in proper working condition. Complete required documentation and daily assignments accurately and timely. Participate in resident activities and engagement opportunities as appropriate. Report all accidents, incidents, safety concerns, or suspected abuse/neglect immediately according to policy. Perform other duties as assigned.
General Working Conditions:
This position requires standing and walking for extended periods of time. While performing the duties of this job, the employee is regularly required to communicate effectively with residents, families, and team members; sit, stand, walk, bend, reach, push, pull, and lift equipment or supplies. The employee must be able to read, write, and speak fluent English. The employee may occasionally lift and/or move up to 25 pounds and assist residents with mobility and transfers. The work environment requires compassion, patience, attention to detail, and the ability to work collaboratively in a fast-paced senior living environment. Occasional travel between communities or departments may be required.
